HOW TO OBTAIN ASHA CEU CREDIT FOR THIS CONFERENCE
(Failure to follow these procedures will result in no credit being awarded to the participant)
1) Obtain the three CE FORMS from the CE table
2) Fill out the following three forms COMPLETELY – (including your ASHA Certification Number)
* CE Participant Form
* CE Learner Assessment/Course Evaluation Form
* CE Conference Evaluation Form
3) TURN in ALL THREE FORMS BEFORE you leave!!
ASHA CEUs are awarded by the ASHA CE Registry upon receipt of the CEU forms from the ASHA Approved CE Provider (EBS Healthcare). Failure to turn in forms prior to leaving this conference will result in no CE credit. Please follow directions carefully.
